Infectious mononucleosis, ruptured spleen and Cullen's sign.

چکیده
Although Cullen' s sign is usually associated with haemorrhagic pancreatitis, this report describes a case in which it occurred in a patient with infectious mononucleosis and non-traumatic rupture of the spleen. Thomas Stephen Cullen (1869-1953) professor of gynaecology at Johns Hopkins University Hospital, Baltimore USA, gave his name to a peri-umbilical ecchymosis resulting from a haemoperitoneum secondary to the rupture of an ectopic pregnancy. Over the years, it has usually been considered to be a sign of haemorrhagic pancreatitis, but it has also been associated with other conditions.' Although a similar case has been published,2 we feel that its occurrence is rare enough to warrant mention, particularly in view of the opinion in a wellknown medical text book that Cullen's sign is so infrequent in splenic rupture that it is academic,3 and a later (17th) edition does not make any mention of an association.

Ruptured internal iliac artery aneurysm presenting as Cullen’s sign
Cullen's sign or periumbilical ecchymosis, is classically considered as an indicator of acute hemorrhagic pancreatitis or ruptured ectopic pregnancy. Internal iliac artery aneurysms are rare and are usually asymptomatic. We present a case of a contained rupture of the internal iliac artery aneurysm presenting with Cullen's sign.

Spontaneous rupture of the spleen is a rare complication of infectious mononucleosis (IM) occurring in 0.1-0.5 percent of patients with proven IM [1]. Although splenectomy has been advocated as the definitive therapy in the past, numerous recent reports have documented favorable outcomes with non-operative management. A review of the literature suggests that non-operative management can be succ...

Cullen's sign, ecchymosis of the subcutaneous periumbilical tissue often described in association with non-malignant conditions such as ruptured ectopic pregnancy or acute pancreatitis, has been reported in malignancies involving the abdomen. In melanoma, hematoma-like metastasis has been observed and can resolve with an effective therapy. We observed resolution of Cullen's sign (probably hemat...
متن کاملRupture of the spleen in infectious mononucleosis.
By MAJOR E. B. SMITH, M.C., AND LT. COL. R. P. CUSTER, MC. I NFECTIOUS mononucleosis is properly regarded as a benign disease. When death occurs it can be ascribed to a complication rather than to the disease itself. Pneumonia, edema of the glottis, and hemorrhage from a deep tonsillar ulceration were terminal conditions in a few cases of which we have personal knowledge. Rupture of the spleen ...
متن کامل[Spontaneous rupture of the spleen in infectious mononucleosis].
Rupture of the spleen in infectious mononucleosis is a rare but potentially fatal complication. Once injured, the "pathologic" spleen may have limited reparative capabilities. In this situation, operative management seems to be the safer therapeutic approach.
